Student Registration

KINDERGARTEN REGISTRATION

Kindergarten Registration is held annually in March at the elementary school building the child is scheduled to attend. If you are unsure which school your child will attend, please call the Administration Center at (708) 532-6462.

To attend school in the Fall of 2013, children must be five (5) years old on or before September 1, 2013.

When registering a Kindergarten student, please bring the child's birth certificate with the county seal affixed and the following documentation:

Category I:  (one document required)

  • Current closing papers
  • Signed and dated lease with two rent receipts
  • Mortgage statement or payment book/coupon
  • Most recent property tax bill

Category II:  (two documents required showing proper address)

  • Driver’s License/State ID
  • Voter’s Registration card
  • Current utility bill - gas, electric, water, cable, phone (not cell)
  • Public Aid card
  • Vehicle Registration
  • Homeowner’s or renter’s insurance premium receipt

It is also necessary to bring the parent’s work phone number and an emergency contact phone number.

For children born in Cook County, birth certificates may be obtained from the Cook County Clerk's Office at www.cookcountyclerk.com or by calling 866-252-8974.

NEW STUDENT REGISTRATION

During the school year, registration will be accepted at the school your child will be attending Monday through Friday from 9:00 a.m. - 11:00 a.m. During the summer months, registration will be accepted at the school your child will be attending on Tuesday and Thursday between 8:00 a.m. - 11:00 a.m. and between 1:00 p.m. - 2:30 p.m. We are closed all Fridays in July.  If you are unsure which school your child will attend, please call our Administration Center at (708) 532-6462.

As Needed, To Prove Residency:

A family living with an in-district resident and seeking enrollment for their child(ren) should download and print our Affidavit of Residency Packet.

RETURNING REGISTRATION

Registration for the 2013-14 School Year - Registration packets will be sent home with students in late April.  Packets should be returned to homeroom teachers.

1. Registration Packets will be sent home with students in late April.
2. In order to receive the discounted rate, Student Fees must be paid by May 10th.
3. Student Fees received after the May 10th deadline will not be eligible for discounted fees and parents will be charged the normal fee rate.

Please note: Students will not be guaranteed their home school if registration forms are returned after the June 30th deadline given in the registration packet.

Returning Student Registration fees can be paid online or can be mailed in or dropped off at your child's school or at our Administration Center. If you wish to pay your returning student(s) fees online, please visit our Web Store by clicking the Online Payments button below:

2013 SUMMER ACADEMY REGISTRATION

A wide variety of Summer Academy classes will be available to our students during the second and third weeks of August. For a detailed description of course offerings and registration information, please click on  Summer Academy Information.

Registration forms and payment are due by Wednesday, May 29, 2013.  Summer Academy instruction will be provided by KSD 140 teachers. Enrollment is limited; if necessary a random drawing will be held to determine class rosters. Each session is $24 and online payments may be made through the District’s Web Store or by personal check made payable to “Kirby School District 140.”

For each session your child would like to attend, please submit a separate registration form with your online receipt or check. If we cannot accommodate your child’s enrollment, your credit card will be credited or the check will be returned.

AT-RISK PRESCHOOL SCREENING

Kirby School District 140 provides an at-risk screening for children who will be three years of age or older by the date of the screening. At-risk screening is provided on selected dates throughout the school year. This screening covers areas of speech, language, fine motor development, gross motor development, and concept information. To make an appointment please have your child's Social Security Number available and call the Department of Special Services at 708-532-8537.

Free Milk/Lunch and Fee Waiver Applications

HEALTH FORMS

The State of Illinois requires the following for all students enrolled in public or private schools:

Early Childhood and Project Pro - Students need a school physical examination.
Kindergarten - Students need a school physical, dental, and vision examination.
2nd Grade - Students need a dental examination.
6th Grade - Students need a school physical and dental examination.

Additionally, any student enrolling for the first time in Illinois must have an eye exam.
